Enforcement Decree of the Employment Insurance Act, Article 128 (Investigations for Hearings)

① 법 제94조제1항에 따른 심사청구에 대한 심리를 위한 조사의 신청은 다음 각 호의 사항을 적은 문서로 하여야 한다.

② 심사관은 법 제94조제1항에 따라 증거조사를 한 경우에는 증거조사 조서를 작성하여야 한다. 이 경우 법 제94조제1항제1호에 따라 심사청구인이나 관계인으로부터 진술을 받을 때에는 진술조서를 작성하여 첨부하여야 한다.
